Child Sex Abuse

It is one of the most devastating acts and is recognized on a significant scale worldwide. The WHO has declared it a “public health priority”.

Children are very vulnerable and sensitive to such accidents and such offenses have a very complex negative effect on development, physical, mental and social well-being.

Pederasty, the use of children for sexual gratification, was highly regarded in ancient Greece because it was believed to give immortality to one’s sexual power.

It was in the late nineties that Freud and Bernard first identified it as a factor associated with ill health. Abraham and Ferenczi showed that the trauma of sexual abuse has a causal contribution to psychiatric morbidity.

DEFINITIONS:

Wingwood defined it as the experience of forced sex before the age of 16.

Kellogg considers any type of sexual touch that is coercive, unwanted and uncomfortable for the child.

Beck-Sague understands it as sexual intercourse with a child through force, threat or intimidation.

Bageley understands this as unwanted touching or caressing of a child’s genitalia or attempted/achieved penetration of a child’s body before the age of 17.

Shechter and Roberge define the involvement of dependent and developmentally immature children in sexual activities that they do not fully understand or to which they are unable to give informed consent and that violate the social taboos of family roles.

Some researchers distinguish between “contact” and “non-contact” forms of abuse… more serious connotations such as penile/digital penetration into the vagina or anus, oral-genital contact and caressing and touching of children’s breasts or genitalia (contact type); while mere exposure of genitalia, making sexual suggestions, using a child for pornographic films or photographs is included in the “non-contact” variety.

Some researchers have described “extrafamilial” and “intrafamilial” types of abuse.

According to Minty, using children for pornography is better understood as “child exploitation” rather than abuse.

Cheasty says childhood sexual contact between siblings must be considered “sexual play” and not abuse.

Macmillan “several researchers believe that INCEST and RAPE require separate identification from child sexual abuse, while others consider them a severe form of contact sexual abuse”.

CHARACTERISTICS OF THE VICTIMS:

The majority of abused children are female with an apparent ratio of 1.5-3:13. The age range varies from 3 to 6 years and from 8 to 11 years.

PERFORMER CHARACTERISTICS:

Considerable evidence shows that abusers are overwhelmingly male. The age of the perpetrators ranges from 20 to 70 years, with the majority in the age range of 25 to 40 years.

Poor academic results, single or divorced or widowed, married with a lack of marital harmony, difficulties in socialization and own victimization are some of the factors that lead to the development of this act somewhere in their lives. A foreign group of perpetrators are individual pedophiles, street drug addicts, individual psychopaths, members of ritual groups. They may suffer from some form of personality disorder, substance abuse disorder, alcoholism psychosis, or senility.

RISK FACTORS FOR SEXUAL ABUSE OF CHILDREN:

Statistically identified risk factors for children are;

(a) the absence of any biological parent, in particular the mother,

(b) feminine,

(c) the presence of a stepfather or foster parent, especially a father,

(d) social isolation,

(e) learning disabilities,

(f) street children,

(g) Poor parental relationship etc.

Similarly, more commonly reported risk factors for adults becoming offenders are:

(a) previous sexual abuse in childhood,

(b) Substance Abuse,

(c) Alcoholism,

(d) social isolation,

(e) masculine gender,

(f) sexual deviations,

(g) personality problems,

(h) Lack of loving parenting in childhood, etc.

CONSEQUENCES OF SEXUAL ABUSE OF CHILDREN:

I. Psychological consequences:

It was found to be a strong predictor of early onset of psychiatric disorders, chronic disease course and slow to poor response to therapies.

Development of behavioral problems such as antisocial behavior, aggressive behavior, truancy, emotional instability, stubbornness, low self-esteem, running away from home, etc.

In adults, there would be eating disorders, suicidal behavior, borderline personality disorder, anxiety disorders, depressive dissociative disorder, post-traumatic stress disorders, sexual dysfunction.

II. PHYSICAL CONSEQUENCES:

Recurrent nonspecific genital, anal, or lower abdominal pain and an increased risk of sexually transmitted diseases, especially AIDS, either early in life or in later life.

III. SOCIAL CONSEQUENCES:

It can be a direct or indirect result of a psychological effect. A significant association was found for high-risk social behaviors such as sex on the first date, multiple sexual partners, consensual and unprotected sex in early adolescence, increasing incidence of teenage pregnancy, early onset of substance abuse, child prostitution, and increasing incidence of adolescent crime. .

The need for extensive research studies is enormous in developing countries such as Pakistan, where environmental conditions suggest its presence at rates higher than those identified elsewhere. Researchers should choose methodologies and instruments for international comparison.
