SNIP/TUCK: Circumcision in the Philippines

In our country, circumcision is not only done for medical purposes. Circumcision was part of our cultural norm; which is expected of every boy at puberty. Pretty weird? It is not like that in our country. You see, circumcision was a ritual, a rite of passage, if you will, that boys had to go through during said stage of life. It is a cultural belief in the Philippines that the transition from boyhood to manhood is not complete without circumcision.

Failure to submit to this traditional Filipino custom will most likely result in ridicule and ridicule from peers and relatives. Those who are not yet circumcised will be invited sopot (intact foreskin) and those who have completed the procedure will look down on it. If someone comes to high school uncircumcised, they will be considered strange and abnormal. Those who reach early adulthood uncircumcised are considered “old” for the procedure. If one does not have a deep understanding and knowledge of Filipino culture, it would be considered a low form of bullying.

THE SEASON OF THE CIRCUMCISION

In the Philippines, there is a so-called circumcision season, and these are the months of April-May, i.e. the summer months. These months are when the children are on vacation; so that, after circumcision, children can rest at home without having to miss school during the healing process. “Operation Tuli” (mass circumcision) is being carried out in various hospitals and health clinics during these months. Hundreds of crying, screaming and fainting boys would line up to undergo this procedure.

PHILIPPINE CUTS

Circumcision is done in two ways in the Philippines – surgical circumcision and traditional circumcision or “Pukpok” (a strong blow or hammering).

Surgical circumcision

A newer, more modern and universally accepted method is surgical circumcision. Surgical circumcision is favored by those living in urban areas where hospitals and clinics are more accessible. It is the circumcision of choice for urban people who are more modernized and educated enough about this type of circumcision.

There are two techniques used for surgical circumcision – the dorsal incision technique and the sleeve technique. A dorsal incision is the technique of choice for those with conditions such as phimosis (tight foreskin) and paraphimosis (non-retractable foreskin). Before circumcision, the foreskin is pulled over the glans. A vertical incision is made towards the middle of the foreskin. An incision is made to the base of the crown of the penis and the skin is excised. This is followed by multiple suturing using absorbable sutures. Of course, all this is done after a local anesthetic injection. This technique was the one used during those times when I assisted with circumcision.

On the other hand, the sleeve technique represents a more aesthetically pleasing penis. Circular incisions are made on the shaft of the penis that reveal the void. The remaining foreskin is then gathered and sewn. This technique is said to be more complex than the dorsal slit technique. This method is suitable for older boys and adults.

Traditional Circumcision or “Pukpok”

This method has caused a bit of controversy over the years due to the nature of the procedure itself. Many have argued that traditional circumcision or “pukpok” is a form of genital mutilation and abuse. This method is done in very rural areas of the country.

Boys would line up to go to the local “circumciser” for this traditional circumcision or “pukpok“. This procedure is preferably done near a river, lake or sea as the boys will be asked to submerge half their bodies (from the waist down) in the water to soften the skin of their penis before going under the knife. They will also be asked to chews guava leaves for later use. “sly“. A razor (labaha) is then placed on top of the foreskin. The foreskin is separated after a few quick blows with the hammer. The boy will then be asked to spit chewed guava leaves onto the wound to stop the bleeding.

I am not sure if this method is still practiced to this day, although there have been reports that it is still practiced.

PHILIPPINE MYTHS ABOUT CIRCUMCISION

Whether it’s a thousand-year-old tradition or not, circumcision is a choice—unless medically indicated. It is the choice of the person being circumcised, not just what one’s culture dictates. Boys should first be informed about the advantages and disadvantages of circumcision. Those who choose not to go with it should never be ridiculed. Finally, traditional circumcision or “pukpok” should be eradicated… completely! Not only is this practice unsafe, unsterile and brutal, it can also psychologically harm boys.
